Original passport (Compulsory) and declaration form if available. Spectacle/ Contact lenses if you wear one. List of current medications / medical records of major operations or illness. Previous Chest X-ray if you have any past history of chest problems. Photos and copies of passport are NOT required. For medical examination you do not have to attend on empty stomach. Please bring a shawl along with you (for female).
